Title:  

Certified Personal Care Aide Level II

Main Function:
  To provide personal care to clients at assigned location / time.
Reports To:   RN Supervisor
Responsibilities:  
  1. Provides varying degrees of assistance to the client with the following tasks in accordance with the established Care Plan for the assigned client.
    • Oral hygiene
    • Grooming
    • Bathing
    • Toileting
    • Walking and/or Transferring
    • Eating
    • Dressing
    • Self-administration of medication
  2. Provides some or all of the following services for the assigned client in accordance with the established Care Plan for the assigned client:
    • Preparing and / or serving meal(s)
    • Changing / making client's bed
    • Dusting and/or vacuuming client's living area
    • Dishwashing
    • Tidying client's kitchen, bedroom and bathroom
    • Laundry and/or ironing for client
    • Assistance with payment of bills and other errands essential to the routine management of the client's home.
  3. Provides other services as authorized by supervisor.
  4. Promotes the maintenance of health and active daily living with clients.
  5. Attends in-service training as required.
  6. Physical condition commensurate with demands of the position.
  7. Must adhere to all Agency confidentiality regulations.
Knowledge, Skills, Abilities and Personal Characteristics:  
  1. Working knowledge of the care of the physical, emotional and developmental characteristics of a client in a home-like setting.
  2. Working knowledge of the modern techniques of nutrition and diet therapy.
  3. Demonstrated ability to transfer client(s).
  4. Demonstrated ability to read, write and perform arithmetic computations at a minimum of an eighth grade level.
  5. Demonstrated ability to understand and follow written and oral instructions.
  6. Ability to maintain positive working relationships with others.
  7. Physical condition commensurate with the demands of the position.
     
Education and Experience:  
  1. A certificate from a training program in personal care services approved by the State Department of Social Services.
    OR
  2. An equivalency certificate from an agency approved by either the New York State Departments of Health or Social Services to provide such a certificate.
    AND
  3. Must have training and knowledge in Age specific competency from birth to death.
