HANCI - Services - Complete Senior Care

On July 12, 2006, the Dale Association and the Health Association of Niagara
County Inc. received a New York State designation to create a Managed
Long-Term Care Program in Niagara County.
Helping Older Adults Remain Independent
Complete Senior Care will help nursing home-eligible older adults to remain
living at home or in the community. In following the PACE model – Program of
All-Inclusive Care for the Elderly – Complete Senior Care will ease the
frustration of older adults and their caregivers by coordinating and
providing all healthcare needs under one umbrella. All care is coordinated
by an interdisciplinary team, whose focus is to anticipate and prevent major
health problems. All services are paid for through capitated, monthly
payments by Medicare and Medicaid, with no co-pays or deductibles on the
part of the participant. Those ineligible for Medicaid may join the program
by paying a monthly fee set by the state government.
Participants will attend an adult day center and clinic regularly for
breakfast and lunch, activities, and a full range of clinical and
therapeutic services. If they need home care on evenings and weekends, it is
provided. The program arranges and pays for hospitalization, prescriptions,
and even short-term and long-term nursing home care if needed. All specialty
care and therapies are included. Spiritual care and social work are provided
as needed.
Medicaid Savings
Complete Senior Care will save public healthcare dollars. Medicare and
Medicaid pay for PACE services per member, per month, rather than the
traditional fee-for-service model. Therefore, the rates for public payers
remain stable, and Medicaid achieves cost savings. Typically, Medicaid
long-term care costs are reduced by 5 to 10 percent in programs of this
kind.
Center Locations and Job Creation
Complete Senior Care will create up to 100 quality healthcare jobs in these
two facilities during the next five years, while providing capital
investment in the cities of Niagara Falls and Lockport. The first Complete
Senior Care center is scheduled to open at HANCI’s campus on Main Street in
Niagara Falls in late 2008; a second PACE Center will open in Lockport in
late 2010. Once both centers are operating at full capacity, the program
will be able to serve 300 clients.
Expertise and Relationships
HANCI and the Dale Association have extensive experience providing home
care, adult day services, senior programs and mental health services. The
agencies have a combined 136 years of experience in serving older adults and
caregivers. The reputations, credentials and collaborative relationships
that these two agencies bring to the table have resulted in a synergistic
partnership.
Sustainability
Once it is operational, PACE will be fully sustainable through Medicare and
Medicaid payments. With sufficient seed money to hire the core staff,
purchase medical equipment and prepare the necessary program space, Complete
Senior Care will no longer rely on grants or contributions for its operating
expenses.
